Lord Jesus, You suffered for the salvation of humanity. You made Yourself Man, that mankind may commune with God. As you walked on earth, you healed the sick, raised humans from the dead, and You freed them from pain. You shared with Your Disciples and the Church the power to heal and to bring deliverance to those taken by illness, in Your Holy Name.
In the name of my eternal faith in You and Your Church, and with the help of Its ordained ministers, I ask that You bring healing to … by the grace of Your Name, my Lord.

Your will be done. All honor and glory to You forever and ever.
“Glorious Archangel St. Raphael, great prince of the heavenly court, you are illustrious for your gifts of wisdom and grace. You are a guide of those who journey by land or sea or ait, consoler of the afflicter, and refuge of sinners. I beg you, assist me in all my needs and in all the sufferings of this life, as once you helped the young Tobias on his travels. Because you are the “medicine of God” I humbly pray you to heal the many infirmities of my soul and the ills that afflict my body. I especially ask of you the favour (here mention your special intention), and the great grace of purity to prepare me to be the temple of the Holy Spirit.
